Functional Medicine New Patient Information

It is important for the doctor to have reviewed your medical history and current symptoms prior to your appointment. To that end, we require all Functional Medicine patients to complete some forms prior to scheduling their no-cost consultation.

1) Download each of these 3 PDF files.

  • FM Health Appraisal

2) Complete the forms. You can print the forms to fill out by hand.  Or, if the PDF reader that you’re using has “Fill and Sign” functionality, you can complete the forms in that manner.

3) Return the form.  Choose the manner which works best for you.

• • Printed forms can be scanned and emailed to patientcenter@innovativehealthclinic.com or faxed to 715-355-4120.
  • Electronically completed PDFs can be saved and emailed to patientcenter@innovativehealthclinic.com

Upon receipt of all forms, the doctor will review your information, and our staff will contact you to schedule your no-cost consultation.  Please allow 1-2 business days to hear from us.
